FCHC OB/GYN is located on the 2nd floor of the FCHC South Medical Office Building on the campus of Fulton County Health Center at 725 South Shoop Avenue in Wauseon, Ohio.

Monday through Thursday: 8:00 AM – 5:00 PM

Friday: 8:00 AM – 4:30 PM.
